What's on the SBAR Card:
Situation
State who you are, who you're calling about, and what the current situation is.
"This is [Name], RN on 3 North. I'm calling about Mr. Johnson in room 312. He is having increased difficulty breathing."
Background
Provide relevant background — admitting diagnosis, relevant medical history, current medications.
"He was admitted 2 days ago for pneumonia. He's on supplemental oxygen at 2L/min and has a history of COPD."
Assessment
Give your clinical assessment of the problem — what you think is happening.
"His O2 sat is now 88% on 2L, down from 94% this morning. Respiratory rate is 26. I'm concerned he is deteriorating."
Recommendation
State what you need or recommend — be specific about what action you are requesting.
"I'd like you to come assess him now and consider increasing his oxygen and possibly ordering a chest X-ray."
